Client’s Consent for Treatment, Consent to Collect and Exchange Personal Information and Disclosure of Pecuniary and Non-Pecuniary Interests

Purpose

This consent form applies to everyone who interacts with Medilinks Access Pty Ltd A.C.N 658 402 352 and Medilinks Connect Pty Ltd A.C.N 675 326 959 (Medilinks).

To deliver the best services in support of your health and wellbeing, we require to work collaboratively with your carers, medical, allied health team, legal advisors and any relevant government departments.

We require your consent so that we can:

  • collect your personal information from the above-named organisations and persons;

  • use and store the information we collected to deliver our services to you;

  • make suggestions to you about our other services that may benefit or enhance your health and wellbeing through our regular newsletters, emails, SMS, or social media platforms;

  • disclose your personal information to these persons/organisations as you authorised us to, so that they can provide recommendations how we can improve or maintain our high-quality services for you; and

If you do not provide your consent to us, we may not be able to provide your required services or to the high standard as we provide to other clients. If you have a particular concern with respect to consenting to the exchange of personal information, we encourage you to speak to Privacy Officer.

We also require your consent so that we can provide you with treatment and care.

Consent to treatment is separate from consent to collect or share your personal information. It means that you understand and agree to the types of healthcare services we may provide to support your mental health and wellbeing.

Before any treatment begins, we will ensure that you:

  • understand what your treatment involves;

  • are aware of any risks, benefits, or alternatives;

  • know your rights to ask questions or withdraw your consent at any time (unless there are legal or medical reasons not to).

This dual consent — for information handling and treatment — ensures that you stay informed, empowered, and actively involved in your care.

Consent For Personal Information

This consent you give to Medilinks will be valid for 12 months unless:

  • you have decided to withdrawal or change this consent;

  • there is a significant change in your health, and it is in your substitute decision maker or Medilinks’ opinion that a new consent is required.

By completing this form, I give my consent to, and full authority for, Medilinks to collect, use and exchange my personal information for the following purposes:

  • conducting the necessary examination and preparing the resulting report or documentation required by relevant third parties including but not limited to the Department of Veteran Affairs, the Commonwealth Super Corporation, Medicare, insurance providers, or other agencies involved in healthcare or billing services;

  • support my chosen quality of life;

  • providing care and support to improve and maintain my health and wellbeing;

  • address or respond to my requests;

  • make suggestions and recommendations to me about Medilinks’ other services;

  • develop and improve the quality and scope of the services Medilinks provide, and seek my feedback;

  • to assist in investigating my complaints, feedback and inquiries;

  • for other purposes which are reasonably necessary in connection with Medilinks’ normal functions and activities, for example, seeking legal/accounting advice, managing business activities, or research;

  • to exchange my personal information with third parties who are involved in my care; and

  • for my information to be collected and used for providing suggestions and recommendations on other services to improve my health or wellbeing.
